Oral health is often overlooked in routine clinic visits. Signs and symptoms affecting the oral cavity can potentially indicate a serious mouth condition or can be a sign of systemic illness.
Bad breath (halitosis) is usually accompanied by other oral conditions, such as dental caries and periodontal disease. Promoting good oral hygiene practices can help alleviate halitosis in most patients.

Halitosis can also be a sign of disease process elsewhere. The two most common non-oral causes of halitosis are post-nasal drip and gastroesophageal reflux.

Pain in the oral cavity can be due to infection, inflammation, trauma, or even referred pain. The most common type of painful ulceration is an aphthous ulcer; a self-limiting condition. However, any persistent area of ulceration warrants a biopsy to rule out malignancy. Oral cancers typically present as an area of irregularity or ulceration that can be painful or painless. Referred otalgia (ear pain) is often an accompanying symptom.

Excessive salivation, or sialorrhea, in patients above the age of four can indicate an underlying pathology. These include:

Anatomic causes, such as macroglossia and dental malocclusion;

Hypersecretion secondary to reflux, inflammation, or medication;

Neuromuscular dysfunction, such as cerebral palsy and Parkinson’s disease

Dry mouth, or xerostomia, is a common symptom in the elderly population. It is often a side effect of polypharmacy, but can also be due to an underlying disease process, such as Sjogren’s syndrome. Patients who have been treated with radiation for head and neck cancers often have a severe degree of xerostomia as well.

Ulcers and plaques in the oral cavity can be cause for concern in some patients. These are often secondary to treatable conditions, such as stress, infection, or vitamin deficiencies. However, any non-healing ulcers, especially in patients who smoke or drink heavily, can be a sign of oral cancer and should be ruled out immediately with a biopsy.

No, a throat swab is not more accurate than a nasal swab in detecting the omicron variant

As more and more people scramble to get tested for COVID, there’s a growing debate on social media about what’s more accurate, a nasal swab versus a throat sample?

Recent social media posts, as well as some doctors, are questioning swabbing techniques as they relate to the omicron variant.

A recent Tik Tok video posted by Allison Hall shows the journalist first receive a negative result after swabbing her nose.

In the video, she says, “Weird ’cause I’m pretty sure I have it, and it’s the omicron variant. I was reading online the omicron variant actually lives in the throat.”

Then, after swabbing her throat, she tested positive, which was later confirmed with a PCR test.

On Twitter, #SwabYourThroat is growing with more people doing the same.

[Fred] My name is Fred Escobedo and I am a cancer survivor.

[Fred] I am the primary caretaker of my children since I have retired. We have five kids. They’re all boys. This is the most difficult job that I had done my entire life.

[Fred] And I’ve been military for 20 years in a combat zone. This is far more difficult than that.

[Fred] I notice like something was in my throat. Like I couldn’t clear it.

[Geoffrey Young, M.D., Ph.D Chief of head and neck surgery, Miami Cancer Institute, in interview setting]

[Dr. Young] Fred ended up having a squamous cell carcinoma which is the most common cancer we see in the head and neck area. In addition, his was caused by the HPV virus, which is now really the most common cancer that we see in the back of the throat.

[Fred] At first, I was in denial that, I don’t smoke I don’t do anything crazy, you know? So why did this happen to me?

[Fred stands by a large window at Miami Cancer Institute, looking outside, deep in thought]

[Dr. Young] We, as physicians, owe it to our patients to look at them, not as their disease but as an individual. We are very lucky at Miami Cancer Institute to be able to provide our patients with multi-disciplinary subspecialty access specifically in the head and neck. We see patients together, myself the medical oncologist, radiation oncologist speech-language pathologist, dental oncologist, when needed will also join the group.

[Dr. Young] The good news is with this particular type of cancer it actually responds well to treatment, but we have to individualize which treatment is going to be best for the patient in the long run.

[Fred] I had received radiation therapy and chemotherapy.

[Dr. Young] The patients who receive specifically radiation and chemotherapy in the head neck it can be very difficult journey for them.

[Fred] It was hard to eat. I lost quite a bit of weight.

[Dr. Young] To have the support services that are able to provide nutritional support. Pain management is very important for our patients.

[Fred] Everyone was very caring. The team at pain management was extremely good. Nutrition people were always trying to find different ways for me to, to keep myself healthy. Everything was in one location. It was very convenient and very helpful.

[Dr. Young] Subsequent imaging was concerning for a residual small amount of cancer in the back of his throat.

[Dr. Young] Fred was lucky in that we now have access to trans oral robotic surgery which is a minimally invasive way of getting to some tumors in the back of the throat. And we were able to remove his tumor through his mouth with the surgical robot. He spent one night in the hospital and went home the next day.

[Fred] I am cancer-free right now. The day I rang the bell I felt it was very emotional. I felt relief that the treatment was completed and I was hopeful for the future. I would like to see my kids grow up, see my grandkids and I feel blessed to have a second chance in life.

Throat cancer survivor remained positive throughout his treatment

Damion Smith is known for his deep, resonant voice. So when he lost is voice in June 2016, he decided to visit his doctor and he was diagnosed with laryngitis. But the medications didn’t work, and his voice was getting worse, so he went to an ear, nose and throat specialist (ENT). He found a growth on Damion’s left vocal cord and took a biopsy. The biopsy revealed cancer and Damion was referred to MD Anderson where he was diagnosed with throat cancer – specifically, primary squamous cell carcinoma of glottis.

After seven chemotherapy cycles and 35 days of radiation, Damion credits his doctors, nurses and staff for getting him through this tough journey, but also credits keeping a positive attitude.

In this video, we’ll talk about some common causes and importantly at the end, I’ll tell you as a doctor what are the key things to watch out for.

As a Family doctor/GP one of the bread and butter conditions we see is sore throats. I often get asked how to get rid of a sore throat? What are some good home remedies for a sore throat? How to treat a sore throat quickly? In this video, I try and simplify the main things to look out for with the presentation of sore throats for students and clinicians. I also present some simple treatments that can be used to help get rid of a sore throat.

Sore throat (pharyngitis) is super common. It is usually caused by an infection in the throat (Viral or Bacterial). Soreness in the throat may be the only symptom. In addition, you may also have:

⦿ Tiredness.
⦿ Swollen glands in your neck.
⦿ Pain when you swallow.
⦿ A hoarse voice.
⦿ A mild cough.
⦿ A high temperature (fever).
⦿ A headache.
⦿ A feeling of wanting to be sick (nausea).

We will then look at how to treat a sore throat yourself:
To help soothe a sore throat and shorten the severity of the symptoms, you can:

1) Gargle with warm, salty water (half a tea spoon of salt) (children should not try this) – one of the most effective and inexpensive ways to try this home treatment. Anybody can prepare this as we demonstrate in the video how to do it.

2) Drink plenty of water – when we have an infection our body will be fighting it and this can send the body in to overdrive. The key is to keep it hydrated and avoid dehydration as this can make us feel unwell.

3) Eat cool or soft foods – Helps create a soothing effect on the throat.

4) Warm drinks like tea and soup may also help (Avoid Hot Drinks) – Again as above, generally cooler foods are preferred but if you don’t like bananas or yogurts then a warm glass of tea or soup can do the trick.

5) Avoid smoking or smoky places – these can irritate the throat further and make your symptoms worse.

6) Suck ice cubes, ice lollies or hard sweets – but do not give young children anything small and hard to suck because of the risk of choking.

7) Rest – Helps the body fight the infection.

Consider seeing a GP/Family Doctor if:
⦿ The sore throat does not improve after a week
⦿ Often get sore throats
⦿ Worried about your sore throat – it is severe/ or you feel unwell
⦿ Have a sore throat and a very high temperature, or you feel hot and shivery
you have a weakened immune system – for example, because of diabetes or chemotherapy
A severe or long-lasting sore throat could be something like strep throat (a bacterial throat infection).
